Where and when did the "renaming of Judea to Palestine" claim originate? What source was used?

Even pro-Palestinians take for granted that Hadrian changed the name of Judea to Palestine to spite the Jews after the Bar Kokhba revolt. Many net entries source Cassius Dio's "Roman History" book 69, but this only tells of the revolt, not of the name change- let alone why it was changed.
